We want to hear from persons with a disability, family members, friends, caregivers, and other stakeholders living in Arizona. Your feedback is vital! The goal is to help AZSILC assess how programs and services currently meet the needs of people living with disabilities in Arizona.

The mission of AZSILC is to promote equality, inclusion, and choice for people who have disabilities through collaboration and public policy change. Survey results provide us with valuable information necessary to collaborate with statewide partners to create new initiatives and to enhance existing services for Arizonans who have disabilities. Note, that this survey is anonymous and no personal identifiable information is required.
